Forms required for physical:
- Signature Form (pdf) - Must print, sign and bring to physical
- Emergency/Insurance Form - Must print, sign and bring to physical
- Copy of front and back of insurance card
- 'Pre-participation' form instructions are listed below.
ATS Portal Info
Type the word 'new' as your Athlete ID
and password in the ATS Portal. You create your own Athlete ID and password once you are in the ATS Portal.
We do not create passwords for you.
Complete the following tabs in the Portal:
- General
- Insurance
- Please bring a copy of the front and back of your insurance card to your physical.
- Contacts
Click 'Save Athlete Info' at the bottom.
Once this is done, a FORMS tab will appear at the top.
- Click on FORMS.
- Go to the Form Name dropdown box.
- Form name is 'Pre-Participation':
- Click new;
- Complete health history,
- Print and bring to your physical.
